COUNTY OF PIERCE REQUEST FOR QUALIFICATIONS 26-001-ADR-HRSN Health-Related Social Needs Pierce County Human Services (PCHS) Aging and Disability Resources (ADR) is seeking applications from qualified organizations to provide Health-Related Social Needs (HRSN) nutrition support services under Washington's 1115 Medicaid Demonstration waiver. Eligible organizations may apply to provide Medically Tailored Meals (MTMs) and/or Fruit and Vegetable Provisions to eligible Medicaid recipients who are not currently receiving in-home long-term care support services (Non-LTSS Clients). Organizations must be qualified for each service they propose to provide and must be able to serve the entire geographic area of Pierce County. This Request for Qualifications (RFQ) is for qualifications only and does not guarantee or promise a contract with PCHS. Organizations that meet the qualifications may be offered an opportunity to negotiate a contract for eligible services.
