Legal Notice Radiology Associates of San Luis Obispo ("RASLO") announces a data incident that may have affected a limited number of individuals' information. Around March 13, 2025, RASLO became aware of unusual activity in certain employee email accounts and immediately began an investigation. The investigation determined that certain information stored in the accounts may have been accessed by an unauthorized party between February 3, 2025, and March 17, 2025. Therefore, they conducted a comprehensive review of the information potentially affected. The type of information likely varies by individual but may include name and one or more of the following: address, telephone number, email address, date of birth, Social Security number, driver's license/state ID number, passport number, U.S. alien registration number, financial account information, payment card information, treatment/diagnosis information, dates of service, treatment location, prescription information, provider name, biometric data, medical record/patient ID number, Medicare/Medicaid number, health insurance information, and/or treatment cost. In an abundance of caution, they are offering individuals access to credit monitoring and identity protection services. For more information or to enroll in these services, individuals should contact the company's assistance line at 1-833-456-5383, Monday through Friday, from 5:00 AM to 5:00 PM PT, excluding holidays. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
