Notice of Proposed Settlement of Granite Wellness Centers Class Action If you believe your personal information was impacted by the Granite Wellness Centers data security incident in 2021, you may be eligible for a cash payment from a $725,000 Class Action Settlement. What is this about? A settlement has been reached in a class action lawsuit about a data security incident that occurred on or about January 5, 2021, which potentially exposed personally identifiable information (“PII”) including names, dates of birth, home addresses, dates of care, treatments, treatment providers, health information, health insurance information, driver’s license numbers, medical histories, Social Security numbers, and bank account numbers. Plaintiffs allege that GWC should be liable for the Data Security Incident because GWC should have taken greater care to protect PII from potential exposure. GWC denies these claims and any wrongdoing. If the Court approves the settlement, individuals residing in the United States whose personally identifiable information or protected health information was accessed or acquired without authorization during the Data Security Incident may submit a claim for cash benefits, including either: (i) a pro-rata cash payment estimated to be approximately $750 subjection to proration (the “Alternative Cash Payment”); or (ii) reimbursement of up to $5,000 for unreimbursed Documented Losses fairly traceable to the Data Security Incident (instead of the Alternative Cash Payment). Settlement Class Members who were California residents at any point between January 5, 2021, and April 26, 2026, may be entitled to an additional $100 cash payment. Any cash amount actually awarded may be prorated depending on the total value of claims made by all Settlement Class Members. What are the benefits? The settlement provides the following benefits: Alternative Cash Payment: All claimants who elect this option will receive a pro rata cash payment estimated to be approximately $750.00, subject to upward or downward proration depending upon the total value of claims made by all Settlement Class Members. If you elect this payment, you do not need to submit any other documentation with your claim form, but you will be unable to claim Documented Losses. Documented Losses Reimbursement: Up to $5,000.00 for documented out-of-pocket losses or expenses. If you choose this option, you will not be eligible for the Alternative Cash Payment. This amount may be prorated depending on the total value of claims made by all Settlement Class Members. California Statutory Cash Payment: If you are a resident of California (and/or who resided in California at any point between January 5, 2021, and April 26, 2026), you may be entitled to an additional $100.00 cash payment, regardless of whether you claim the Alternative Cash Payment or Documented Loss Reimbursement. You must file a Claim Form to receive a cash payment.
