See form 470 # 260005837 at USAC's E-rate Productivity Center that can be accessed at https://www.usac.org/e-rate/resoures/e-rate-productivity-center/. Vendor must be a participant of the E-rate discount program, understand all of the rules, billing steps, etc., and have a valid SPIN#.
