Media Notice: Incident Notification Security and privacy are top priorities for Insurance Office of America ("IOA"), and IOA is committed to protecting the information entrusted to it. IOA recently provided notice of an incident that may have involved some personal information, including protected health information, of certain individuals who participated in one or more IOA benefit plans during the 2024–2025 plan year. On May 14, 2026, while sending a routine benefits-related email to a limited number of IOA benefit plan participants, certain personal information was inadvertently disclosed. Upon discovering the error, IOA immediately took steps to address it, including initiating the recall process within minutes of the original email being sent and promptly commencing an internal investigation. IOA's investigation determined that the information included individuals' names and certain other personal information, including protected health information, names, Social Security numbers, and health insurance and benefit plan enrollment information. Not all data elements were present for every individual. IOA notified affected individuals by mail on June 22, 2026. Although IOA has no information to date indicating that the information involved in this incident was or will be used for any unintended purposes, to help relieve concerns and restore confidence following this incident, IOA has arranged for affected individuals to enroll, at no cost, in complimentary credit monitoring and identity protection services for 24 months through Epiq. Affected individuals who received individual notification letters should refer to those letters for specific enrollment instructions. IOA remains committed to protecting the information of its benefit plan participants. To help prevent something like this from happening again, IOA has implemented enhanced safeguards, including additional training for its team and strengthened review procedures for potentially sensitive outbound communications. IOA is also continuing to review and further enhance its existing security protocols and practices.
