The City of Hialeah (“the City”), is seeking proposals from all qualified firms to provide basic group term life and supplemental term life insurance with accidental death and dismemberment coverage. The City seeks proposals for a thirty-six (36) month basic group term life and supplemental term life insurance with accidental death and dismemberment coverage. Bids received after said date and time or in some other manner than as stated above will not be considered and no time extensions or waivers will be granted.
