Project Description: The applicant proposes to establish a new medical home health agency to be known as Infinite Care Solutions, LLC. The agency will provide Medicare-certified home health services, including skilled nursing care, physical and occupational therapy, and medical social services. Service Area: The proposed project will provide services to patients in Sumter, Richland, Lexington, Kershaw counties.
