A completed Subcontractor/Vendor Questionnaire including latest year-end safety and financial information must accompany the proposal unless previously submitted. Elaborate proposals or the inclusion of non-essential information e.g. marketing/promotional material, safety manuals, resumes, etc. is strongly discouraged.
