You are further notified that when a child has been placed in the temporary and/or legal custody of the Idaho Department of Health and Welfare for fifteen (15) out of the most recent twenty-two (22) months the Department shall, prior to the last day of the fifteenth month, file a petition for termination of parental rights unless the child has been permanently placed with a relative, there are compelling reasons why termination of parental rights is not in the best interest of the child, or the Department has failed to provide reasonable efforts to reunify the child with his family.
