Advertisement for Proposal The City of Lake Worth will accept sealed proposals for Employee Group Health, Dental, Vision and Life/AD&D Insurance until 10:00 a.m., October 30, 2025. Late proposals will not be accepted and will be returned unopened. Proposals are requested for insurance products only, not agents or consultants. Two (2) copies of the sealed proposal will be accepted until the date and local time listed at the City Hall Office, 3805 Adam Grubb Rd., Lake Worth, Texas. Each proposal must be in a sealed envelope and labeled "City of Lake Worth Employee Group Medical, Dental, Vision and Life/AD&D Insurance" on the outside of the envelope. One (1) copy of the Specifications and any other Request for Proposal documents may be obtained on or after October 10, 2025, from the City Hall Office, 3805 Adam Grubb Rd., Lake Worth, Texas. 76135.
