PUBLIC NOTICE
Public Notice of Intent to Destroy Student Records
In compliance with state and federal regulations, Madison CUSD12 is providing notice of its intent to destroy certain student records that are no longer required to be maintained.
Reason for Destruction:
To comply with records retention schedules and ensure the appropriate disposal of information that is no longer needed for educational services or district operations.
Records Scheduled for Destruction May Include:
Temporary student records (e.g., health records, disciplinary records, special education records, etc.)
Records of students who graduated, transferred, or withdrew on or before August 2018.
Important Information for Parents/Guardians and Former Students: Parents/guardians of eligible students, or students who are 18 years of age or older, may request a copy of these records prior to destruction. These records may be needed for future educational, employment, or personal purposes.

After the deadline, records will be securely destroyed in accordance with state and federal law
